Please read and complete the applicable form with as much detail as possible. You may submit the form by email to forensic@miottawa.org or drop off at the front desk of the Ottawa County Sheriff’s Office (12220 Fillmore Street) during regular business hours. Once the form is reviewed, you will be contacted regarding your request.
