I/DD Adult Services
To request mental health services, call the Access Center at 616-393-5681 Monday through Friday between 8am and 5pm. If you would like to facilitate beginning services, you may call or walk in during business hours.
Eligibility
The person must have a severe, chronic condition that:
- Is attributed to a mental or physical impairment or a combination of both
- Started before age 22
- Is likely to continue indefinitely
AND
The severe/chronic condition results in substantial functional limitations in 3 or more of the following major areas:
- Self-Care
- Receptive and Expressive Language
- Learning
- Mobility
- Self-Direction
- Capacity for Independent Living
- Economic Self-Sufficiency
