Aging can bring challenges, but you don’t have to face them alone. Senior Reach® is here to support residents aged 60 years or older in Ottawa County with compassionate, home-based care and connections to community resources.

Built By a Trusted Network & Community Support
Senior Reach® is guided by an Advisory Board of Directors consisting of representatives from Life Circles, Senior Resources, Evergreen Commons, MDHHS Adult Protective Services, The Alzheimer Association, Four Pointes, and local law enforcement. Together, they ensure Ottawa County seniors receive the care and support they deserve.
This program began in 2016 as a pilot project with grant funding from the Michigan Association of Community Health Boards and Ottawa County residents subsequently approved millage funding to support this evidence-based approach. This funding is set to expire in 2026.
